To register your referral, we’ll need your name, date of birth, contact details including postal address, and the name of the GP surgery you’re registered with.

    The form should only take about 5 minutes.

  • How to access the service by phone

    Call us on 0333 200 1893 during these hours*:

    • 8am – 8pm Monday to Friday
    • 9am – 12.30pm Saturday


    We’ll take some details from you and arrange an appointment.

    *We’re also available out of hours. One of our counsellors will provide you with immediate emotional support and take referral information from you.

Deciding to seek support and help can be a big step and it’s important that you know what to expect when you access the service.

What's next?

We’ll book you in for a telephone assessment. Before your assessment, we’ll send you (by email or post) some questionnaires. Please fill these in before your appointment; this will help the therapist making the call.

Telephone assessments usually take 1 hour. Please try to find a private, quiet place beforehand.
