Why have I been referred for this health check?
You are entitled to this health check due to a mental health diagnosis you have. Having this health check once a year means that we can check your physical health in line with your mental health. We complete this healthcheck on behalf of your GP practice.
What is included in the health check?
Why isn’t the healthcheck being held at my own GP?
We are sometimes able to hold clinics at your registered surgery, but when we are unable to, we will book you into a clinic location that is nearest to your address and GP.
How long will the healthcheck take?
It will take around 15 – 20 minutes
Do I need to fast for this healthcheck?
No, you do not need to fast for your healthcheck as blood glucose and cholesterol is taken by a finger prick.
What if English isn’t my first language?
If you do not feel comfortable conversing in English, a telephone interpreter can be provided for your healthcheck. We will organise this for you with the cost covered by the NHS. Please notify our team in advance of your appointment if a interpreter is required.
What if I need to rearrange or cancel?
Please call 01268977171 as soon as you know you will be unable to make an appointment and need to rearrange. We request that cancellations be made at least 24 hours prior to the appointment. Should you need wish to cancel over the weekend or out of hours, please leave a message on our answer machine.
What if I am running late?
If you are running late for an appointment, please contact us so that we can inform the clinician of an estimated time. To ensure you are given sufficient time in your appointment you may be asked to reschedule to another day and time.
